4. Informed Consent Form in English and Tamil (for prospective study)
5. Confidentiality statement and Application for waiver of consent
(for retrospective study, chart review & stored samples)
6. Assent form & Parental consent form (for participants in the age group 13 – 18
years)
7. Parental consent form (for children in the age group 0 – 12 years)
8. Proforma / Questionnaire
9. CV of all investigators
10. Itemized budget
